Most common causes
- Benign Paroxysmal Positional Vertigo (BPPV) — dislodged otoconia in the semicircular canals. Short, intense vertigo triggered by head movements. Treatable with repositioning maneuvers (Epley, Semont).
- Vestibular neuritis — inflammation of the vestibular nerve, often post-viral. Intense continuous vertigo for days.
- Ménière disease — recurrent vertigo attacks with fluctuating hearing loss, tinnitus and ear fullness.
- Vestibular migraine — vertigo linked to migraine, sometimes without headache.
- Cervicogenic dizziness — imbalance related to neck tension and proprioception.
- Orthostatic / autonomic dizziness — blood-pressure or metabolic origin.
In-consultation assessment
The consultation includes a detailed history, otoscopy, cranial-nerve screening, positional tests (Dix-Hallpike, Roll test), nystagmus evaluation and gait/balance assessment. When clinically indicated, additional testing with video head impulse test (vHIT), audiogram or vestibular rehabilitation is arranged.
Epley maneuver for BPPV
When BPPV is confirmed, the Epley maneuver can be performed in the same appointment. Most patients notice significant improvement after one or a few sessions. Home exercises and positional advice complement the in-office treatment.
Integrative dimension
Chronic dizziness can also be driven or worsened by sleep quality, blood-pressure control, chronic inflammation, metabolic factors (iron, B12, vitamin D) and stress/anxiety. When appropriate, these are integrated into the assessment — a strength of this clinic alongside the classical ENT workup.
